I will try....Batter is called out, runner on third called out on an attempt to steal home plate, runner on second, thinking that was the third out runs off the the field and is tagged the catcher. Thus, unassisted triple play.

That would be an unassisted triple play but the catcher is fielding the ball when making tags.
I'm guessing interference of some sort. :dunno:
Guess your definition of fielding throws me for a loop. 1st, the ball never entered fair territory. What exactly did the catcher field. I would think catching balls/strikes would not be fielding because it is not computed in fielding pct. nor as chances. So in your def of fielding don't think this can happen. Agree?
I'm gonna take a stab at this:
Strike out (1 Out) throw 'em out (2 out) throw out another runner trying to advance. (3 out)

Because the infield fly rule is called, yes the batter is automatically out for the first out. And yes, what pool hustler said with trail runner (runner on 1B) passing the lead runner (runner on 2B) for out number two. When the infield fly rule is engaged, the other runners on base can advance on their own accord. Thus, our lead runner goes back to second base and tries to tag up and advance to third base but he is somehow called out on interference with the fielder covering second base while in the midst of trying to tag up.... A triple play without the ball being fielded.
However, if it is fielder obstruction based on the umpire's decision, then the umpire would award our lead runner third base. Therefore, it would only be a double play and not a triple play.
Either way, it would look like mass confusion on the basepaths... be it a double play or a triple play.

@Panther. When a catcher catches a 3rd strike he is credited with a putout and with men on base the ball is still in play. The catcher also gets charged a fielding error when he allows a ****** ball.

batter hits a grand slam, none of the runners run, the batter passes all three? Not sure if the lead runner is out or the trail runner. Or some combination of runners passing the runner ahead of them...

Not a grand slam unless there are 4 runs scored. Once the ball is it it is in play. In this case, 1 RBI by hitter.
