No SS# needed since you're essentially a freelancer. However, we will need some personal info for our records. This is mostly real name, address, phone number, birthdate and email. This information is 100% confidential and handled with upmost care, as if you were here working at FO HQ.

The only thing that will ever show publically is your first name. If it's very unique then of course I'll allow you to take a common name like Mike or Bob.
