Moving away from the kinky shemale fetish aspect, if a person chooses to undergo surgery to change their gender & all that that entails I would presume they feel pretty strongly that they are not the gender they happen to have been born to. I would argue that these people are certainly a strong case for the fact that gender is more than a simple question of physical attributes but is also a state of mind.

If they still have a cock, their gender is unchanged and are still male. Getting a pair of bolt-ons does not change gender. I would even argue that full op trannies have not changed their gender, as they do not have female reproductive organs.
If gender also involves state of mind, is it not possible for them to mentally come to terms and accept what they were born as, instead of the psychological confusion and artificial alterations. Making surgical changes and taking female hormones are not going to make them a woman.
