Phobias refer to irrational fears only. For example, the fear of mice crawling next to you is a phobia since they are incapable of hurting you. A fear that they might contaminate your food is a rational fear.
Similarly, it is a phobia if you are afraid to shake the hand of someone with HIV/AIDS, but not a phobia if you get pricked with a needle that has been used to inject them.
