There are very good fakes, but never any perfect ones. The fact is, not only is perfection not guaranteed, but the may not even be better than what you had already! I'll stick to natural, but it is a case-by-case sorta thing.

I don't like fake ones if it's just for the cause to make them bigger, but better fake ones than saggy ones.
If you have big boobs, and they start so sag, better make them smaller than put silicone to make 'em "stand" if you necessarily feel you want to change them.
