I kinda doubt that it was an "accident". if people were doing things to her that she didn't sign up for (and that she has refused to do before) prior to shooting, I think it would really hurt the reputation of the director or the studio producing the film, not to mention that she would probably stop the scene and refuse to work with them anymore.
