In the case of that video, it's probably a trained skill. No, he's not doing a true multiple-orgasm, he's brought himself to the point of barely needing to cum and then letting it leak out a little at a time, rather than stroking himself intensely to try and shoot the whole load all at once. The key is not to stroke off (or otherwise be stimulated) immediately prior to, during, and in the seconds after the cumshot.
