Remember couples years ago, all the plastic surgeons were very busy taking out all the silicone implants.
They have to do a lot of re-constructive plastic surgery including dissecting excessive skinfolds and also removal of the breast tissues.
The problems are very large amount of excessive skinfolds needed to be dissected and the "nipples" have to be removed and placed in a much higher location.
The plastic surgeon will always do a "Breast lift" to tighten the skinfold and place a smaller saline implant in some case so it looks a little more perky but less gigantic.
I have seen a woman with removal of her gigantic silicone implant but no re-constructive surgery. You know how the breast looks like. It drops all the way to the pubic area. The whole breast skinfold looks horrible and wrinkled and believe it or not, the breast becomes a two and half feet skinfold dropped always on the floor ! (actually half-way on the floor, terrible looking) !
