Well, it has a few meanings.
1. Its a diminutive of Herz = heart. So a small or tiny heart. (But normally not the heart in a human body. Just the object like this .... :lovecoupl (<- We would say: There is a "Herzchen" between the two smilies)
2. Its a "pet name". Like your English honey or sweety. But its pretty archaic. Its not used very often anymore.
