I suppose it depends, but it's usually the director. If a performer doesn't want it in a certain place, like facials, she specifies that before agreeing to work with anyone. There's always a list for actors that states what they are and are not comfortable doing.

So directors will look at the list and create the story based around that. Or he'll just find someone who fits his needs for the film.
