I just saw something else on this, recently. I think he bled out 1.5 pints, before the paramedics arrived? I think it was 20 minutes later, and he bit his lip to keep from passing out? My guess is the starlet was on top, and got "too high," he popped out, and she happened to slam down on an angle where it .. "folded over."
