She only had a fat transfer to her breasts.
Her breasts are still natural.
Only breasts with implants are fake.

Fat transfer surgery is still an for of "enhancement" so IMO still fake.

regardless she's a DDD cup now, not her pre surgery D cup.

While she hasn't confirmed it, it looks like she had the same with her butt and that's probably a 42" (personal guess) size now.

Any surgical procedure done to alter the original, natural state of a body part can technically be considered "fake."

While no, they are not implants, it is still taking away the natural look that "god gave her" so yes... fake. Just because you don't consider it as such doesn't make it not so.

Is there a difference between implants and fat transfer? Yes, but both fall into the same category.
