Previously, there were two separate topics for these reports. These have now been merged into a single, catch-all thread. This thread covers everything that could be reported before, with room for additional issues as well.
The idea is that things are kept on top of like the thread fixes thread rather than letting a massive backlog develop like before.
Please use this thread to report any of the following:
Where possible, link directly to the relevant source to save us some time having to look it up ourselves. We also recommend checking multiple sources before submitting a report, unless the information comes from a highly credible primary source (i.e. something the individual in question has said or published online themselves).

- Duplicate profiles (e.g. the same person listed under different names)
- Incorrect profile photos (assigned to the wrong person)
- Inaccurate biographical information (such as height or date of birth)
- Content that has been incorrectly labelled or assigned to the wrong person (photo galleries, videos, or links)
- Incorrect names (or suggestions for a more accurate name)
