By "worst", the author means from a health standpoint. It doesn't mean calories only. All of these dishes are a combination of high calories, fat and sodium. Of course I can't speak for all Americans but I most certainly avoid fat and sodium as much as I reasonably can.

I didn't know he had a second book out. I'll check it out for sure. This is great information as far as I am concerned.
 
I'm of the philosophy that you can eat as many calories as you want as long as you burn them off. The harder you work, the more calories you need. The more sedentary your work is, got to cut the caloric intake.

Yep. It's all about metabolism but fat and sodium are not good for you in large quantities under any circumstances.
